So after a very long day involving my normal vet and then the teaching hospital and a bunch of tests and stuff, here's what's going on with Suri.
Bloodwork results - bun 188, creat 7.3, serious levels of anemia, other stuff I don't remember. No obvious signs of infection or other easily treatable thing.
Ultrasound results - kidneys are small and abnormal and are showing changes consistent with end stages of renal failure. Indications of possible pancreatitis and of a probably-benign lump in the liver that isn't helping matters. Weirdness with a few other organs that could be related to the renal failure or could be lymphoma but at this point it sort of doesn't matter.
Physical exam results - dehydrated and malnourished and generally not in good shape.
Overview - probable end stages of renal failure, possibly complicated by lymphoma. IV fluids would rehydrate her but only as long as the IV is in, and would complicate the anemia; a blood transfusion would help the anemia but only for the life of red blood cells, because she can't really produce her own
I was given a three way choice: a) admit her to the vet hospital for IV stuffs, which is high stress for low gain and about as useful in the long run as a band-aid on a gaping chest wound; b) take her home, keep doing the subq fluids as long as she allows it, keep track of her quality of life and call them when it gets bad enough; or c) euthanize her.
I went with option b.
So she's back home, but I have no idea for how long. The prognosis is pretty damn poor. And all I can really do is love at her.
